调和肝脾法治疗肠易激综合征临床观察

Clinical Observation of Irritable Bowel Syndrome by Harmonizing the Liver and Spleen

目的:以调和肝脾立法,选逍遥散和痛泻要方合方为主加减,观察治疗肠易激综合征的疗效,为中药治疗肠易激综合征提供立法上的依据。方法:对2005年1月-2006年12月笔者的门诊病人,随机分为治疗组和对照组,治疗组以逍遥散合痛泻要方作为基本方,针对个体不同、症有偏颇给予加减;另设西药治疗对照组,观察两组间疗效。结果:治疗组47例,显效25例,有效16例,无效6例,显效率占53.2%,总有效率87.2%;对照组45例,显效10例,有效19例,无效16例,显效率占22%,总有效率为64.4%。两组显效率及有效率比较,经统计学处理均有显著意义(P<0.05)。治疗组疗效优于对照组。结论:中药调和肝脾法治疗肠易激综合征具有较好的疗效,其原理与肠易激综合征的发病多与患者的情绪和精神状态有关,而中药调和肝脾法可能有抗抑郁和抗焦虑作用。

Objective .To provide the evidence of the theory and method for treating the irritable bowel syndrome by Chinese Material Medica, based on treating effect of irritable bowel syndrome by the combination of Xiao Yao San（ Carefree Powder） and Tong Xie Yao Fang（Important Formula for Painful diarrhea） according to harmonizing liver and spleen method. Methods ： Divide the writer＇s out -patient from January, 2005 to December, 2006 into treating group and contrasting group（45 cases）. The treatment group was treated based on Xiao Yao San（ carefree powder） and Tong Xie Yao Fang（ important formula for painful diarrhea） with a slight change according to individual constitution and syndrome while the contrasting group treated by western medicine. The effects were observed. Results ： In the treatment group, 25 cases got good effect while 16 cases has certain effect with 6 ineffective cases, so the effectual ratio is 53.2% and the total effective rate will be 87.2% ; in the contrasting group, 10 cases got good effect while 19 cases has certain effect with 16 ineffective cases thus the effectual ratio is 22% and the total effective rate is 64.4% which make the comparison of effectual and total effective ratio between two groups has the obvious meaning according to statistical analysis （ P 〈 0. 05 ）. The effect of treating group is much better than that in the contrast group. Conclusion ：Harmonizing the liver and spleen method has a good effect in treating IBS for the onset of IBS relates with mood and physiological state of patients while harmonizing the liver and spleen method may be have the effect of antianxiety and antidepression.
