经皮穿刺臭氧注射术治疗腰椎间盘突出症的探讨

Percutaneous treatment of lumbar disc herniation by injection of medical ozone

目的:探讨经皮穿刺臭氧注射术对腰椎间盘突出症的疗效及适应证范围。方法:60例患者经CT或MR检查证实为腰椎间盘突出症,且有相应腰腿痛临床症状。其中椎间盘突出41例,膨出19例;单纯包容性46例,伴有明显腰椎退行性变及椎管狭窄14例。在DSA监视下采用21G穿刺针经皮穿刺至腰椎间盘,实施盘内及椎旁间隙臭氧注射术。盘内注射5 ml,椎旁间隙注射10～15 ml,O_3浓度为35～45μg/ml。结果:随访1～17个月,总有效率为75.0%,其中显效率为61.7%,有效率为13.3%,无效为25%。结论:经皮穿刺臭氧注射术是治疗腰椎间盘突出症的有效方法。

Objective：To evaluate the clinical efficacy of O3 injection in treatment of lumber disc herniation by its indications. Methods ： 60 cases tients with lumbar disc herniation confirmed by CT or MRI were tre with medical ozone. Of the total 60 cases, 41 presented with protrusion and 19 with bulging, 46 eases were simple contained ones and 14 complicated with obvious degeneration of lumbar vertebrae and spinal stenosis The injection was conducted with fluoroscopic guidance by using puncture needle of size 21G. 5ml of 03 with concentration of 35- 45μg/ ml was injected into intervertebral space and 10-15 ml was administered in paraspinal space. Results ： A follow-up of the patients from 1 to 17 months showed that total effective rate was 75. 0%. 37 out of 60 achieved an excellent outcome （ 61.7% ） and the effective rate was 13.3%（8/60）, but 15 out 60 failed to recover. Conclusion： The percutaneous treatment of lumbar disc herniation by injection of medical ozone was a safe and effective method .
