摘要
目的探讨多根多处肋骨骨折的记忆合金肋骨接骨板治疗手术指征、手术方法、疗效。方法在全麻下经后外侧切口进行对10例多根多处肋骨骨折记忆合金肋骨接骨板治疗,其中6例浮动胸壁患者,肋骨骨折数5～10根,胸廓塌陷变形4例,合并肺挫裂伤血气胸5例;腰椎骨折脱位伴截瘫3例,胸腹联合伤者3例。结果10例多根多处肋骨骨折患者均行记忆合金肋骨接骨板,术后胸痛显著减轻,胸壁稳定,呼吸功能恢复,痊愈出院,术后2个月复查,无明显胸廓塌陷畸形,X线胸片示内固定牢固,肋骨骨折端骨性愈合。结论对于严重多根多处肋骨骨折应用记忆合金肋骨接骨板治疗,手术简单、手术时间短、固定牢固、疗效显著。
Objective To approach the results of internal fixation of multiple fractured ribs with memory alloy rib bone plate and discuss the indications and the technical points of the operation.Methods Under general anaestheia,10 cases of fractures of multiple ribs were treated by memory alloy rib bone plates through standard thoracic incision.Among 10 cases with fractures of multiple ribs,the number of fracture of rib was 5-10,6 patients with followed flail chest,4 with chest deformity,5 with pulmonary contusion and hemopneumothorax,3 with fracture dislocation of lumbar vertebra and paraplegia,3 with combined thoracoabdominal wound.Results After internal fixation,chest pains were obviously relieved,chest walls were stable,respiratory functions recovered and then patients recovered.During 2 months′ follow-up,chest deformity was invisible,and X-ray indicated stability of internal fixation and healing of fracture points of rib.Conclusions Severe collapsed chest wall or Flail chest caused by fracture of multiple ribs should be treated by memory alloy rib bone plate and memory alloy rib bone plate is a simple,firm and effective method.
