慢性肾脏病C反应蛋白与肾功能及同型半胱氨酸的关系 被引量:3

摘要 目的:探讨3~5期慢性肾脏病(CKD)患者的微炎症状态与肾功能及血清同型半胱氨酸水平的关系。方法:分组:CKD组为3~5期CKD住院患者共51例;健康对照组为22例肾功能正常的健康体检者,另将CKD组按血清总同型半胱氨酸(tHcy)水平分为高tHcy组(血清tHcy≥15μmol/L)和非高tHcy组(血清tHcy<15μmol/L)。高敏C反应蛋白(hs-CRP)采用免疫比浊法测定;应用荧光偏振免疫分析法(FPIA)检测tHcy;肾小球滤过率(GFR)采用预测方程(MDRD简化公式)计算。结果:CKD组GFR水平为(14.8±12.99)ml·min-1·1.73m-2;血清hs-CRP水平为(3.31±3.50)mg/L;tHcy水平为(23.25±13.11)μmol/L。对照组血清hs-CRP水平为(0.95±0.84)mg/L;CKD组血清hs-CRP水平明显高于对照组(P<0.01);CKD组患者血清hs-CRP水平与GFR无相关性;高tHcy组和非高tHcy组相比血清hs-CRP水平无差异(P>0.05),CKD组血清hs-CRP与tHcy水平无相关性。结论:3~5期CKD患者存在微炎症状态,尚不能认为CKD患者高同型半胱氨酸血症诱导炎症反应发生;3~5期CKD患者血清hs-CRP水平增高与肾功能减退程度并不平行,3~5期CKD患者微炎症状态这个复杂过程中有多因素参与。
