马凡综合征心血管病变的外科治疗

SURGICAL MANAGEMENT OF CARDIOVASCULAR ASPECT IN MARFAN' S SYNDROME

采用Bentall术式治疗13例马凡综合征。其主动脉瘤为DeBakeyⅠ型2例。Ⅱ型11例．均伴有重度主动脉辩关闭不全。手术死亡1例（7．7%）．为感染性心内膜炎致死。余12例无严重并发症。随访1~29个月，情况良好，作者对手术适应证、术中心肌保护、术中采用价循环上胜静脉道灌技术、预防感染及防治术后心律紊乱等问题进行讨论。

Between Dec. 1991 to Apr. 1997, thirteen patients ofMarfan's syndrome underwent Bentall operation. Aortic a-neurysm was classified as DeBakey Type I in 2 cases, TypeⅡ 11, all with severe aortic insufficiency. Mortality was7. 7 % (1/13), one died of infective endocarditis. No signif-icant complications oceurred in other 12 cases. Follow - upwas 1 ~ 29 month, no late death repsorted. Indication , my-(x'ardial Protection, circulatory arrest with retrograde cerebralperfusion via superior vena cava, prevention of infeetion andpost - operating arrhythmia were discussed.
