无痛分娩中硬膜外阻滞麻醉的护理措施

Nursing measures of epidural analgesia for labor pain

目的探讨分娩镇痛的效果及对产程、母婴的影响及护理措施。方法采用PECA泵硬膜外腔给药用于分娩镇痛,观察产程时间、分娩方式、产后出血、胎儿宫内窘迫及新生儿窒息情况。结果观察组和对照组产程比较、两组分娩方式比较有显著性差异(P<0.01,P<0.05);两组胎儿宫内窘迫,新生儿窒息及产后出血发生率比较,无显著性差异(P>0.05)。结论PECA泵用于持续硬膜外腔给药镇痛,疼痛阻滞完善,加速了产程的进展,降低了剖宫产率阴道难产率,对母婴均无不良影响。护理使分娩镇痛达到最理想的效果,采取有效的护理手段,真正地为孕产妇服务,从心理、生理、社会和环境上为她们创造舒适的心境、健康的身体以及顺利自然分娩的内外环境。

Objective： To study the pain relief effectiveness and its influence on the labor course and both mothers and newborns.Methods： Pump of PECA（patient controlled epidural analgesia） was used during labor in the experiment group.The labor course,the mothods of delivery,the incidence of postpartum hemorrhage,fetal distress and neonatal asphyxia were observed in the experiment and control groups respectively.Results： There were significant differences between both groups in stages of labor and delivery modes（P〈0.01,P〈0.05）.There was no significant difference in the incidences of postpartum hemorrhage,fetal distress and neonatal asphyxia（P〉0.05）.Conclusion： Pump of PECA during labor is safe and effective,which accelerates duration of labor and reduces rates of cesarean and dystocia.Nursing enables the childbirth analgesia to achieve the most ideal effect.Effective measures should be taken to truly serve the pregnant woman psycholofically,physiologically and socially in creating the proper circumstances for their comfortable mood,the physical health and smooth delivery.
