胃术后吻合口溃疡的预防和治疗(附24例报告)

Prevention and Treatment of Stomal Ulcer in Gastrectomy(with 24 cases report)

目的探讨胃术后吻合口溃疡发生的主要原因和预防及治疗。方法回顾分析1995～2004年我院收治的24例吻合口溃疡患者的临床资料。结果17例获再次手术切除原吻合口和胃空肠Roux—en—Y吻合。术中发现首次手术的存在问题。结论首次手术处理不当是发生吻合口溃疡的主要原因。原吻合口切除和胃空肠Roux—en—Y吻合是治疗吻合口溃疡的有效手段。

Objective To investigate the main reasons of stomal ulcer after gastrectomy, as well as prevention and treatment. Method The clinical data of 24 cases of stomal ulcer admitted from 1995 to 2004 were retrospectively analysed. Results 17 cases had second stomal resection and gastrojejunal Roux - en - Y anastomosis. Problems in the first operation arose in the second operation. Contusion The mistakes made in the first operation are the main cause for stomal ulcer. Stomal resection and gastrojejunal Roux - en - Y anastomosis are effective methods to treat stomal ulcer.
