某院老年科感染患者病原菌分布及耐药性监测 被引量:5

摘要 目的研究某院老年科感染患者的病原菌分布及其对抗菌药物的耐药情况。方法分析该院老年科2006~2007年所分离培养出来的细菌菌种,根据2007年CLSI颁布的准则,采用WHONET5.4软件完成统计分析。结果该院老年科感染患者临床分离的2799株菌中,革兰阴性菌71.06%,革兰阳性菌9.43%,真菌19.51%。61.91%的金黄色葡萄球菌和51.39%的凝固酶阴性葡萄球菌对头孢西丁耐药,但无万古霉素耐药株;6.98%的肠球菌属对万古霉素耐药;大肠埃希菌和克雷伯菌属对头孢菌素高度耐药,酶抑制剂使用使耐药率下降了54%~68%。铜绿假单胞菌对亚胺培南耐药率为30.26%。真菌对大部分的抗真菌药都呈现敏感,但对特比耐芬高度耐药。结论通过对细菌的耐药监测和分析了解各病原菌的耐药特点与机制,指导临床早期经验性用药。 Objective To guide the .clinical drug use by studying the distribution and drug resistance of pathogens isolated from senile patients. Methods All the pathogens isolated in 2006 - 2007 were used for the assessment of antimicrobial susceptibility, and the results were analyzed with WHONETS. 4 software. Results There axe 2 799 strains isolated from senile patients, including 71.06 % Gram-negative bacilli, 9. 43 % Gram-positlve cocci and 19.51% fungi. 61.91% of Staphylococcus aureus and 51.39 % of other staphylococci were resistant to Oxacillin ; 6. 98% of Entercoccus were resistant to Vancomycin. The resistance rates of Escherichia coli and Klebsiella to cephalosporin were significantly high. 30.26% of Pseudomonas aeruginosa were resistant to Imipenem. Most of the fungi were sensitive to antifungal drug, but resistant to Terbinafine. Conclusions Antibiotics should be rationally administrated with more considerations to the characteristics of epidemiology and drug resistance profile of the pathogens in the given department of senile patients.
