类风湿关节炎住院妇女焦虑抑郁症状及相关因素

ANALYSIS OF ANXIETY-DEPRESSION SYMPTOM AND THE RELATED FACTORS AMONG FEMALE IN-PATIENTS WITH RHEUMATOID ARTHRITIS

[目的]探讨女性类风湿关节炎(Rheumatoid Arthritis,RA)住院病人心理健康状况及其相关因素。[方法]采用无记名调查问卷的方式,应用焦虑自评量表(SAS)和抑郁自评量表(SDS),对在合肥市3家医院风湿科住院接受治疗的240名妇女进行焦虑、抑郁症状评定,并对社会支持、婚姻调适、应付方式等因素进行调查。[结果]焦虑、抑郁检出率分别为37.5%和72.7%。文化程度在初中及以下、居住在农村、家庭人均月收入在800元以下、身体功能在2级及以上、关节有畸形、晨僵时间大于半小时的RA病人比文化程度在高中及以上、居住在城市、家庭人均月收入在800元以上、身体功能为1级、关节无畸形、晨僵时间小于半小时的病人焦虑和抑郁症状检出率均高。年龄在49岁以上、既往无正规治疗的病人抑郁症状检出率分别高于年龄在49岁以下、既往有正规治疗的病人。焦虑、抑郁与疼痛、应付方式、婚姻调适、身体疲劳及社会支持相关联。经Logistic回归分析表明,进入焦虑症状的方程的变量为屈服、疲劳;居住在农村、文化程度低、晨僵时间长、屈服性应付方式、婚姻调适不好和社会支持差可增加抑郁症状的危险性。[结论]在为RA病人进行治疗时,应当关注她们的心理健康状况。

[Objective] To explore the mental health status and its related factors among female inpatients with rheumatoid arthritis. [ Methods] The anxiety and depression of 240 female patients with rheumatoid arthritis in the departments of rheumatism of three hospitals in Hefei were evaluated with SAS questionnaire and SDS questionnaire by anonymous investigation. And the social support, marriage adjustment and coping style of these patients were investigated. [ Results] The detection rates of anxiety and depression were 37.5% and 72.7%, respectively. The detection rate of anxious and depression among patients with education of junior high school or below, living in the rural areas, family month income per capita below 800 yuan, health function above two grade, joints with abnormity and morning stiffness time above half an hour were significantly higher than those with education of high school or above, living in urban areas, family month income per capita above 800 yuan, heahh function above one grade, joints without abnormity and morning stiffness time below haft an hours. The detection rate of depression among patients with the age above 49 years old and previous non-regularity treatment were significantly higher than that among patients with age below 49 years old and previous regularity treatment. Anxious and depression have significantly association with pain, coping style, marriage adjustment, fatigue and social support. The results of Logistic regression demonstrated that surrender, fatigue could add the risk of anxiety, and living in rural areas, education level, long time of morning stiffness, submission style, bad marriage adjustment and social support could increase the risk of depression. [Conclusion] Attention to the mental health status of patients with RA should be strengthen during the treatment.
