经阴道彩色多谱勒对声像图不典型的异位妊娠的诊断价值

Application Value of Transvaginal Ultrasonography for Diagnosing Atypical Ectopic Pregnancy

目的探讨经阴道彩色多谱勒(TVCDS)对声像图不典型的异位妊娠的诊断价值。方法对手术和病理检查已确定为异位妊娠的63位患者的超声声像图进行分析,从中选出未发现妊囊的附件包28例,并对其二维图象、血流参数进行回顾性分析。结果63例患者中35例患者超声声像图可在附件区见到孕囊样回声;28例未见妊囊样回声,其中18例表现为一侧附件区比较规则的长条形包块,10例表现为一侧附件区形态不规则的包块,19例在包块附近或周边探测到比较丰富的血流信号,9例未探及血流信号。结论妊娠实验阳性者,宫内未发现孕囊回声,经阴道彩色多谱勒超声发现附件包块,根据二维图象及CDFI显示的包块内或周边血流动力学改变,结合病史,对声像图表现不典型的异位妊娠的诊断具有一定的可靠性。

Objective The purpose of this study was to evaluate the clinical value of transvaginal uhrasonography for diagnosing the ectopicpregnancy .with atypical sonogram.Methods 63 cases of ectopic pregnancy affirmed by post-operative pathology were involved retrospectively . 28 of 63 cases with atypical sonogram were studied on 2D images of adnexal masses, blood flow signals and spectral features . Results The sonograms in 18 patients showed long strip of adnexal masses and the other 10 patients showed irregular shape of adnexal masses, 19 patients could be detected color flow signals and 9 not . Conclusions Patients with positive pregnancy test sonogram showed no intrauterine gestation but found adnexal masses,the shape of the mass combine with the character of the hemodynamic change are helpful for diagnosing ectopic pregnancy.
