CT动态扫描在肠系膜上静脉梗死中的诊断价值

Diagnostic value of helical CT dynamic scan in superior mesenteric vein infarction

目的探讨CT动态扫描在肠系膜上静脉梗死中的诊断价值。方法回顾性分析13例肠系膜上静脉梗死病人,分析平扫、门静脉期增强扫描及延迟扫描的CT表现特点,并与超声及手术后病理结果对照。结果肠系膜上静脉梗死患者表现肠系膜上静脉内见圆形充盈缺损,增强显示肠管扩张、肠壁增厚、靶征、肠壁强化程度减低等肠缺血的CT表现。CT平扫肝脏呈等密度;增强扫描后。门脉供血减少,门静脉期延迟,肝脏一过性血供不足,肝实质门脉期呈不均匀强化,延时扫描造影剂在肝脏内分布均匀。结论CT动态扫描在肠系膜上静脉梗死诊断中具有重要价值。

[Objective] To explore the diagnostic value of helical CT dynamic scan in superior mesenterie vein （SMV） infarction. [Methods] 13 patients with superior mesenteric infarction of vein had retrospective analysis by CT plain scan, portal vein enhancement scan and delayed scan, to analyze CT appearance and compare with ultrasound and postop pathology consequence. [Results] Round filling defect was found in superior mesenteric vein, intestinal canal broadening, intestines wall thickening, target sign, strengthen degree degrading et al. Mesentery was engorged and dropsyed by plain scan in patients with mesenterie superior infarction in trunk vein corroborated by clinical pathology. Liver displayed equidensite, and blood supply in portal vein was insufficient in enhancement scan. Portal vein phase was delayed, blood supply of liver was insufficiency and liver parenehyma was inequable strengthened in the arterial phase. Delayed scan display eonstrast medium distributing uniformity in liver. [ Conclusion] Helical CT dynamic scan plays an important role in the diagnosis of SMV thrombosis.
