阴沟肠杆菌喹诺酮耐药基因的检测 被引量:4

摘要 目的调查阴沟肠杆菌中喹诺酮耐药基因(qnr)的携带状况,为临床治疗提供依据。方法使用VITEK-AMS鉴定病原菌;qnr与超广谱β-内酰胺酶(ESBLs)耐药基因型检测采用聚合酶链反应(PCR)扩增技术;耐药基因水平传播采用质粒接合试验;根据CLSI指南进行药敏试验。结果55株阴沟肠杆菌中,23.6%qnr基因检测阳性,其中46.2%的qnr基因阳性菌株对环丙沙星敏感,qnr基因阴性菌株的敏感率为83.3%;38.2%的产ESBLs菌株qnr基因检测阳性,2株qnr基因阳性菌株接合试验成功。结论产ESBLs阴沟肠杆菌中常同时携带qnr基因,qnr基因可以进行菌株间的水平传递。 OBJECTIVE To investigate the appearance of quinolone resistance gene(qnr)in Enterobacter cloacae for clinical treatment.METHODS E.cloacae was identified by VITEK-AMS;detection of qnr and extended-spectrum β-lactamases(ESBLs)resistant genes were performed by PCR.Plasmid conjugation test was used to determine the transmission of qnr.The antibotic susceptibility of E.cloacae was detected according to the CLSI guideline.RESULTS The incident rate of E.cloacae with qnr was 23.6% in 55 strains.The sensitive rate to ciprofloxacin was 46.2% in isolates with qnr,but it was 83.3% in isolates without qnr.qnr Genes of 38.2% ESBLs isolates were positive.Two strains harboring qnr genes were conjugated successfully.CONCLUSIONS ESBLs strains usually harbor qnr genes.qnr Genes can be horizontally transferred to recipient by plasmid conjugated test.
