髁支持钢板配合拉力螺钉治疗股骨远端粉碎性骨折

摘要 目的总结髁支持钢板配合拉力螺钉治疗股骨远端粉碎骨折的临床效果。方法18例股骨远端粉碎性骨折患者采用髁支持钢板及拉力螺钉内固定,股骨髁间用2~3枚松质骨螺钉做拉力固定,较小骨折块用拉力螺钉固定在较大的骨折块上。骨折按AO/ASIF分类:A3型2例,C1型4例,C2型8例,C3型4例。结果平均随访13个月,17例骨折正常愈合,1例延迟愈合,平均骨折愈合时间4.5个月;按Kolmener评定标准:本组优7例,良9例,一般1例,差1例,优良率88.9%。膝关节活动度为54°~118°,平均96°。结论髁支持钢板配合拉力螺钉能有效治疗股骨远端粉碎性骨折,具有固定牢固、骨折愈合率高、并发症少等优点。 Objective To summarize the treatment effect of distal femoral comminuted fractures utilized with condylar steel plate matched with tensile screw. Methods A total of 18 cases of fractures were treated by condylar steel plate and tensile screw. Intercondylar fractures were fixed by 2-3 screws, and the small fracture pieces were fixed on bigger fracture pieces. The fractures were classified according to AO/ASIF: 2 case of A3, 4 cases of C 1, 8 cases of C2 and 4 cases of C3. Results With averaged 13 months follow-up, 17 fractures healed, and 1 case healed delayed, with mean union time of 4. 5 months. According to the standard of Kolmener: 7 cases of excellent, 9 cases of good, 1 case of common, and 1 case of poor, the rate of good reached 88.9%. The average knee range of motion was 96 degrees. Conclusion The condylar steel plate and tensile screw were effective for the treatment of distal femoral fracture.
