摘要
目的研究基质金属蛋白酶-9(MMP-9)和环氧合酶-2(COX-2)在宫颈癌组织中的联合表达及其相关性。方法采用免疫组化SP法检测72例宫颈浸润癌(ICC)、16例癌旁正常宫颈上皮(NCE)中MMP-9和COX-2的表达情况,分析MMP-9和COX-2与ICC病理分级、临床分期、间质浸润和淋巴结转移等临床病理特征的相关关系和偏相关性质。结果MMP-9和COX-2在宫颈癌组织中的表达强度呈正相关(rs=0.823,P=0.000)。MMP-9的表达强度与临床分期(rs=0.252,P=0.033)、病理分级(rs=0.343,P=0.003)和淋巴结转移(rs=0.370,P=0.001)呈正相关;相对于临床分期而言,病理分级是影响MMP-9表达更为关键的因素。COX-2表达强度与淋巴结转移(rs=0.249,P=0.035)和间质浸润程度(rs=0.278,P=0.018)呈正相关,但淋巴结转移是影响COX-2表达更重要的因素。结论MMP-9和COX-2在宫颈癌细胞增殖、浸润和转移中有协同作用。联合检测二者的表达对于进一步理解宫颈癌的生物学行为,判断宫颈癌的恶性程度、转移潜能和预后有指导意义。
Objective To and their correlation. Methods investigate the expressions of MMP-9 and COX-2 in cervical carcinoma Immunohistochemical SP method was used to detect the expressions of MMP-9 and COX-2 in 72 cases of invasive carcinoma of cervix (ICC) and 16 cases of normal cervical epithelia remote from tumor (NCE) , The relationships between the expressions of COX-2 and MMP-9 in ICC and some factors relating to clinical pathology of cervical carcinoma, such as histopathological grading, lymphnode metastasis, stroma involvement and FIGO staging, were analyzed statistically. Results The rates of possitive expressions of MMP-9 and COX-2 in ICC were singnificantly higher than those in NCE. The expression of MMP-9 was positively correlated with that of COX-2 in ICC (rs =0. 823, P=0. 000). The expression of MMP-9 was positively correlated with lymph node metastasis (rs =0. 370, P=0. 001), FIGO staging (rs = 0. 252, P = 0. 033) and histopathological grading (rs = 0. 343, P = 0. 003). Histopathological grading was more important than FIGO staging in affecting the expression of MMP-9. The expression of COX-2 was positively correlated with lymph node metastasis (rs=0. 249, P=0. 035) and stroma involvement (rs=0. 278, P=0. 018). Lymph node metastasis played a more important role than stroma involvement in affecting the expression of COX-2. Conclusion The overexpressions of COX-2 and MMP-9 are closely related to the invasion and growth of cervical carcinoma. COX-2 and MMP-9 cooperate the proliferation, invasion and metastasis of cancer cells. Detecting the coexpressions of COX-2 and MMP- 9 may be of value in further understanding the biological behavior and predicting the prognosis of cervical carcinoma.
