摘要
目的探讨环氧化酶-2(COX-2)、血管内皮生长因子(VEGF)、CD105抗体标记的微血管密度(MVD)与乳腺癌的病理生物学特征及预后的关系。方法收集了我院1999-2006年有完整存档蜡块资料的117例乳腺癌蜡块以随机分配的原则组成组织微阵列,用S-P法对上述三个指标的表达情况进行了检测,并且对其表达情况与疾病的生物学特点、患者的生存时间进行了分析,以判断其对预后的意义。结果117例乳腺癌蜡块中COX-2阳性表达65例(55.6%),COX-2在肿瘤中的表达与肿瘤临床分期(P=0.01,r=0.238)、淋巴结转移(P=0.0006,r=0.326)、HER-2(P=0.025,r=0.218)及雌激素受体(ER)(P=0.026,r=-0.205)状态具有密切关系;VEGF同样在乳腺癌组织中有高表达(66.7%),并且其与COX-2的阳性表达水平与患者生存时间有密切关系;CD105抗体标记的MVD值在复发组和未复发组无明显差别(P=0.148),但与COX-2、VEGF均呈正相关(P=0.023,r=0.201;P=0.006,r=0.210)。结论1.COX-2、VEGF在乳腺癌中均有高表达,皆是预后不良指标。2.乳腺癌组织中COX-2的过表达可能是通过促进VEGF来参与肿瘤血管生成从而导致乳腺癌侵袭、转移的。
Objective To find the correlations among the microvessel density (MVD) of cyclooxygenase-2 (COX-2), vascular endothelial growth factor (VEGF) and CD105 antibody labeling and the pathobiologic characteristics and prognosis of breast cancer. Methods Paraffin blocks of 117 eases of breast cancer had been collected in our hospital from 1999 to 2006, which were microarrayed at random and detected by the S-P method. The expression of COX-2, VEGF and CD105 were detected and the relationships among above three expressions, the biological behavior, and the survival time were analyzed. Then their prognostic signigicance was judged. Results Among the 117 breast cancer cases, the expression of COX-2 was positive in 65 eases (55.6%). The correlation between COX-2 expression and TNM stage, lymph node metastasis, Her-2, ER state were significant in statistics. High expression of VEGF was also found in breast cancer tissue (66.7%). The survival time of patients with positive COX-2 and VEGF expression was significantly shorter than those without high expressions. The MVD value of CD105 antibody labelling showed no noticable difference between the relapse group and the non-relapse group (P=0. 148) but, itwas positively correlated with COX-2 and VEGF (P=0.023, r=0.201); (P=0.006, r= 0. 210). Conclusion 1. The high expressions of COX-2 and VEGF detected in breast cancer tissue predict poor prognosis. 2. The overexpression of COX-2 that leads to the invasion and metastasis of breast cancer may be participting in the formation of cancer vessels by means of promoting VEGF.
