基质金属蛋白酶-9在甲状腺良恶性病变组织中表达及临床意义

Study on the Expression of Matrix Metalloproteinases-9 in Benign and Malignant Thyroid Tissues

目的:探讨甲状腺良恶性病变组织中基质金属蛋白酶-9(MMP-9)的表达及其临床意义。方法:应用微波-LSAB免疫组化法检测50例甲状腺癌、45例甲状腺腺瘤和20例癌旁甲状腺组织中MMP-9的表达。结果:甲状腺癌中MMP-9表达阳性率(64.0%)显著高于甲状腺腺瘤(37.8%)和癌旁甲状腺组织(25.0%,P<0.05)。MMP-9表达阳性率与甲状腺癌组织类型无关;有淋巴结转移者显著高于无淋巴结转移者(P<0.05);病理分期Ⅲ～Ⅳ期病例显著高于Ⅰ～Ⅱ期病例(P<0.05);MMP-9阳性的甲状腺癌复发及病死率显著高于阴性者(P<0.05)。结论:MMP-9表达对甲状腺癌恶性程度判断、生物学行为预测和预后评估有一定价值。

Objective：To investigate the expression of matrix metalloproteinases-9 （MMP-9）in benign and malignant thyroid tissues and its clinical significance. Methods：The expression of MMP-9 in 50 thyroid cancers,45 adenoma and 20 adjacent noncancerous portion tissues were respectively studied with microwave-LSAB immunohistochemical methods. Results：The positive rate of MMP-9 in thyroid cancer was 64. 0% , which was significantly higher than 37.8 % in adenoma and 25.0% in adjacent noncancerous portion （ P 〈 0.05 ）. No correlation was observed between expression of MMP-9 and histological grading of thyroid cancer. The expression of MMP-9 in lymph node metastasis and grade Ⅲ -Ⅳ were both higher than that in negative cases and in grade Ⅰ - Ⅱ （ P 〈 0.05 ）. The percentage of recurrence and death in MMP-9 positive cases were notably higher than that in negative cases（ P 〈 0.05 ）. Conclusion ：The expression of MMP-9 can be regarded as a parameter for evaluating the degree of malignancy, biological behavior and prognosis of thyroid cancer.
