超声检查对甲状腺结节内钙化灶的显示及临床意义 被引量:13

摘要 目的探讨超声检查对甲状腺结节中不同钙化灶的诊断价值。方法观察70例甲状腺病变内不同钙化灶在良恶性病变中的检出率,比较探头频率7.5 MHz和11 MHz检测微钙化的不同结果。结果所有病例均经手术后病理证实,钙化灶在甲状腺癌中的检出率为47.4%高于在甲状腺良性病变中的25.5%(P<0.05)。微钙化在甲状腺癌中的检出率为36.8%高于甲状腺良性结节的9.8%(P<0.05)。粗钙化和弧形钙化在甲状腺良恶性病变中的检出率无统计学差异(P>0.05)。使用频率11 MHz较7.5 MHz能更清晰地显示甲状腺癌内微钙化的形态。结论钙化在甲状腺癌和甲状腺良性病变之间的差异多集中在微钙化上,探头频率11 MHz较7.5 MHz更易于显示病灶内更小的微钙化。 Objective To investigate the diagnostic value of high- frequency ultrasonography (include 7.5MHz and llMHz) in identifying differenttype calcification in thyroid carcinoma. Methods Seventy patients underwent high- resolution ultrasonography (7.5 MHz or 11 MHz) to detect the calcification in thyroid nodules. Results The detection rate of calcification ( microcalcification, coarse calcification or peripheral calcification) was higher in thyroid carcinoma than that in benign nodule ( P 〈 0.05 ), especially microcalcification( P 〈 0.05). High frequency ultrasound (11 MHz) showed high sensitivity in detecting microcalcification. Conclusion The microcalcification has great value in the differential diagnosis of thyroid carcinoma and benign thyroid nodule. High frequency ultrasonography (11 MHz) can more effectively detect and identify the thyroid microcalcification.
