乳腺癌X线征象与微淋巴管生成的相关性研究

摘要 目的:探讨浸润性乳腺癌的X线征象与血管内皮生长因子-C(VEGF-C)表达及微淋巴管密度(MLVD)的相关性。方法:回顾性分析资料完整的105例浸润性乳腺癌X线征象,应用免疫组化技术检测标本中VEGF-C、MLVD的含量,分析二者的相关性。结果:105例乳腺癌中68例VEGF-C阳性表达(占64.76%),其中55例高表达(占80.88%),MLVD平均为36.75±15.12;VEGF-C阳性组MLVD为39.63±12.25,VEGF-C阴性组MLVD为16.12±6.51,两组间差异有显著性意义(P<0.05)。乳腺癌X线征象:肿瘤直径≥2.0cm、毛刺征(+)、血管影增多增粗,腋窝淋巴结肿大与VEGF-C表达及MLVD含量密切相关(P<0.05)。结论:浸润性乳腺癌的部分X线征象可评价其组织中VEGF-C、MLVD的表达及微淋巴管生成情况,进而一定程度预测乳腺癌的恶性程度、转移及预后。 Objective:To discuss the correlation of mammographical signs with the expression of vascular endothelial growth factor C (VEGF-C) and microlymphatic vessel density (MLVD) in infiltrative ductal carcinoma of the breast. Methods:The plain film data of 105 cases with infiltrative ductal carcinoma of breast proved by pathology were retrospectively analyzed. Immunohistochemical technique was used to measure the expression of VEGF-C and MLVD in these tissues,and the correlation of imaging signs with them was analyzed. Results: VEGF-C were expressed in 68 of 105 patients (64.76%) ,including 55 (80.88%) cases with higher expression. MLVD was 36.75 ± 15.12, and it was 39.63±12.25 in the breast cancer of positive expression of VEGF while 16.12±6.51 in the breast cancer of negative expression of VEGF. The difference was remarkable (P〈0.05). Some imaging signs of breast cancer,including size of lesion (≥2.0cm), spiculated sign (+), vascularity, and axillary lymph node metastasis were closely related to the expression of VEGF-C and MLVD. Conclusion.. Some imaging signs of infiltrative ductal carcinoma of breast could reflect the expression of VEGF-C and MLVD and biological behavior of tumor cells. Therefore it could play a definite role in forecasting the prognosis and metastasis of breast cancer.
