特异性IgG、IgM定量检测在小儿肺炎支原体感染中的临床意义 被引量:10

摘要 目的探讨肺炎支原体(MP)特异性IgG、IgM定量检测在小儿MP感染中的临床意义。方法采用SERI- ON定量酶联免疫吸附试验检测218例呼吸道感染患儿双份血清特异性MP-IgG、IgM,同时检测82例患儿家长血清特异性抗体。结果各年龄组呼吸道感染患儿第1份血清MP感染阳性率分别为:婴儿组8.9%,幼儿组29.4%,学龄前组35.1%,学龄组47.8%,IgM总阳性率为23.9%,IgM总阳性率为17.0%。有20例患儿的第1份血清MP呈阴性,而第2份血清为阳性,双份血清的MP感染阳性率差异有统计学意义(P<0.05)。52例MP感染患儿家长中,27例血清特异性MP-IgG、IgM呈阳性;30例非MP感染患儿家长中,8例血清特异性MP-IgG、IgM阳性,两者比较,差异有统计学意义(P<0.05)。结论在小儿呼吸道感染中,MP感染阳性率随着年龄的增长而增加,双份血清IgG、IgM的定量检测有助于提高MP感染诊断的正确性。 Objective To study the value of quantitative mycoplasma (MP)-specific IgG and IgM antibodies in the diagnosis of children MP infection. Methods Paired serum samples were taken from children with respiratory tract infection and specific MP-IgG, IgM antibodies were measured by SERION quantitative ELISA. Sera taken from Eighty-two parents were also detected by SERION quantitative ELISA. Results The first paired serum samples of the 218 children with respiratory tract infection and 8 samples in 90 cases were MP-IgM positive (8.9%) in group of infancy, 20 samples in 68 cases were MP-IgM positive(29.4%) in group of toddler's age,13 samples in 37 were MP-IgM positive (35.1%) in group of preschool age, 11 samples in 23 were MP-IgM positive (47.8%) in group of school age, total MP-IgM positive were 52 (23.9%), the positivity rate of MP infection was different at various ages. With 20 cases, the first serum MP detection was negative, but the second serum detection was positive, the at various rate of MP infection was different for two paired samples. The parents sera quantitative MP-IgG, IgM detection showed: samples from 27 out of 52 parents with MP infection children were MP positive,8 out of 30 parents without MP infection children were MP positive, the difference was significant. Conclusion In respiratory, tract infection children, the positivity rate of MP infection increases with age. Quantitative MP-lgG, IgM detection with paired serum samples is of importance in increasing the specificity and sensitivity of the diagnosis of MP infection.
