摘要
目的研究急性脑梗死患者超早期血清基质金属蛋白酶(MMPs)水平与磁共振(MR)改变的相关性。方法对47例发病6h内的急性脑梗死患者进行头颅MR检查和血清MMP-2、MMP-9及MMP-13水平检测。结果脑梗死患者血清MMP-2、MMP-9及MMP-13的水平分别为(242.70±78.42)ng/ml、(113.40±108.65)ng/ml及(84.31±47.62)pg/ml。血清MMP-2、MMP-13水平与MR改变无相关性(均P>0.05),MMP-9水平与MR的平均通过时间图(r=0.371,P=0.026)、达峰时间图(r=0.379,P=0.023)、脑血流量图(r=0.447,P=0.006)的异常体积及大血管闭塞(r=0.416,P=0.004)有正相关性。结论血清MMP-9水平在急性脑梗死超早期与脑MR改变呈正相关,而血清MMP-2及MMP-13水平与脑MR改变无相关性。提示MMP-9参与了急性缺血性脑损害的病理过程。
Objective To evaluate the correlation between the ultra-early serum matrix metalloproteinases (MMPs) levels and the change of magnetic resonance in patients with acute cerebral infarction. Methods The lesion of cerebral infarction was evaluated by magnetic resonance and the levels of serum MMP-2, MMP-9 and MMP- 13 were detected in 47 patients with acute cerebral infarction within 6 hours after onset. Results The average serum levels of MMP-2, MMP-9 and MMP-13 were (242.70 ±78.42 ) ng/ml, ( 113.40 ± 108.65 ) ng/ml and ( 84. 31 ± 47.62 ) pg/ml. MMP-2 and MMP-13 levels were not correlated with the change of magnetic resonance (all P 〉 0.05 ). However, There were significantly positive correlation between MMP-9 level and abnormal volume on mean transit time map ( r = 0. 371, P = 0. 026 ), time to peak map ( r = 0. 379, P = 0. 023 ), cerebral blood flow map ( r = 0.447, P=0.006) and great vessel occlution(r=0.416, P=0.004) on magnetic resonance. Condusions The serum MMP-9 level of acute cerebral infarction are significantly positively correlated with the change of magnetic resonance ,but there axe no correlation between MMP-2 and MMP-13 levels and the change of magnetic resonance. It suggests that the MMP-9 may be involved in the pathology of acute cerebral ischemia.
