神经内镜下前交通动脉瘤显微外科夹闭术

摘要 目的:探讨神经内镜辅助显微外科夹闭前交通动脉瘤的可行性及其意义.方法:回顾性分析内蒙古民族大学附属医院神经外科收治的23例前交通动脉瘤患者,根据术前诊断性影像资料,采用神经内镜辅助显微外科夹闭动脉瘤.结果:21例夹闭完全,2例与周围粘连紧密,采取包裹治疗.术后对病人随访6个月~2年,无再出血发生.结论:利用神经内镜不但可以帮助术者清晰地了解动脉瘤结构,还可以探查到瘤蒂具体位置以及动脉瘤后壁下隐藏的穿通支血管,从而减少了对周围脑组织、重要神经和血管的损伤,降低了手术后并发症的发生率,提高了手术疗效. Objective:To study the feasibility and the clinical meaning of neuroendoscope-assisted microsurgcal occlusion treating aneurysm of anterior communicating artery. Methods:23 cases of this disease treated with endoscope- assisted clipping from Mar 2004 to Mar 2007 were analyzed retrospectively. We analyzed image data before operation. Results: 21 cases were completely cured. 2 cases were treated with pack. No case rebled in the followup of 6 months to 2 years. Conclusion: There is a sufficient exposure to the anatomy of the aneurysm and surrounding structures. The injuries of the operation were relieved and the operative effect was improved.
