可乐定透皮控释贴片降压疗效观察

Evaluation of the Antihpertensive effect of Clonidine Therapeutical Transdermal 24 Hour Ambulatory Blood Pressure Monitoring

目的用２４ｈ动态血压监测方法评价可乐定透皮控释贴片（Ｃ－ＴＴＳ）的降压疗效。方法轻中度原发性高血压患者１５例，平均年龄５２．１±８．４岁。用安慰剂贴片２周后，改用一贴Ｃ－ＴＴＳ（含量２．５ｍｇ／片，面积２．２７ｃｍ２，日释放量０．１ｍｇ），７天更换贴片。结果６例患者仅用一贴Ｃ－ＴＴＳ血压降至目标血压，１２例患者使用１～３贴血压得到控制，总有效率为８０％。治疗后白天、夜间及２４ｈ平均舒张压均明显降低（Ｐ＜０．０５）；白天、夜间及２４ｈ的收缩压和舒张压负荷明显降低（Ｐ＜０．０１）。结论Ｃ－ＴＴＳ能降低２４ｈ血压，作用时间可达７天，治疗依从性好。

Aim\ To evaluate the antihypertensive effect of clonidine therapeutical transdermal(C TTS) by 24 h ambulatory blood pressure monitoring(ABPM).\ Methods\ This trial was conducted according to a single blind, placebo controlled design.\ After a 2 week run in period on placebo patch,15 mild to moderate hypertensive patients aged 39 to 62 years, whose DBP≥95 mmHg on casual measurement, were selected in the trial.\ 24 h ABP and conventional blood pressure(CBP) were measured at the end of the run in period(in a non drug condition), 7th or 14th or 21th day of the application of a 2.27 cm 2 C TTS patch(2 5 mg clonidine per patch) or 2 to 3 patches and on the 2nd day after discontinuation of C TTS patch with an identical placebo patch.\ Results\ One patch of C TTS reduced 6 patient's BP to target pressure(DBP<90 mmHg).\ Total effecacy rate of antihypertensive reached to 80% after one to three patches.\ 24 h mean DBP day and night DBP decreased remarkably(P<0 05).\ Day and night and 24 h SBP and DBP loads(including day and night SBP and DBP loads) reduced obviously as well(P<0 01).\ Conclusion\ The antihypertensive effects of C TTS was evident throughout the day and night, especially on diastolic pressure.
