摘要
目的:Staple钉技术能对脊柱骨骼生长方向进行三维调控从而矫正脊柱畸形。通过观察镍钛合金记忆加压钉置入山羊胸椎实验节段的Cobb角变化,探讨加压钉对脊柱生长调控的效果。方法:实验于2007-03/12在解放军总医院动物实验中心、骨科研究所完成。幼年雌性山羊19只随机分为3组,长钉组7只短钉组7只,空白对照组5只。长钉组和短钉组通过前路手术在胸椎置入记忆加压长钉(7mm齿深)、短钉(4mm齿深),空白对照组不实施干预。分别于术前、术后进行X射线平片检查,使用量角器测量山羊侧位平片上脊柱的Cobb角。结果:①实验过程中长钉组1只发生上呼吸系统感染,死于术后痰液窒息,短钉组1只死于术中出血过多,空白对照组1只免疫预防时死于过敏反应。进入结果分析长钉组6只,短钉组6只,空白对照组4只。②手术前后短钉组的T4~9节段Cob角变化为(8.04±0.69)°,长钉组为(11.35±4.51)°,两组比较,差异无显著性意义(P=0.1059)。③将长钉组与短钉组合并为实验组,实验组术前T4~9节段Cobb角度为(0.50±7.18)°,术后为(10.75±6.72)°,手术前后比较,差异有显著性意义(P〈0.05)。空白对照组在实验开始时T4~9节段Cobb角度为(-10.25±1.26)°,实验结束时为(-10.75±1.50)°实验结束时实验组与空白对照组比较,差异有显著性意义(P=0.007)。结论:正前方置入记忆合金加压钉可以控制山羊胸椎的生长方向造成后凸。
AIM: Staples can regulate spinal growth direction and correct spinal deformity. In this study, we explored the effect of Nickel-Titanium (NT) memory alloy staples in controlling the growth of the thoracic vertebrates in goats by measuring Cobb angle. METHODS: The experiment was performed at the Experiment Center for animals and the Laboratory of Orthopaedics of General Hospital of Chinese PLA from March to December 2007. Nineteen infant female goats were selected and randomly divided into long staple group (n =7), short staple group (n =7) and blank control group (n =5). Long staple (7 ram) and short staple (4 ram) were implanted into long and short staple groups by anterior approach. The control group was not given any treatment. X-ray examination was performed pre- and post-operatively. Cobb angle of lateral radiograph was measured using conimeter. RESULTS: (1)One goat in long staple group died of sputum asphyxiation caused by respiratory system infection, one in short staple group died of excessive blood loss, and one in control group died of hypersensitive reaction during immunoprophylaxis. Finally, 6 of long staple group, 6 of short staple group, and 4 of control group were included in final result. (2)T4-9 Cobb angle was (8.04±0.69)° in short staple group, and (11.35±4.51)° in long staple group after surgery. There were no significant differences between two groups (P =0.1059). (3)T4-9 Cobb angle was (0.50±7.18)° in two experimental groups before surgery and (10.75±6.72)° after surgery (P 〈 0.05). T4-9 Cobb angle was (-10.25±1.26)° in control group before surgery and (-10.75±1.50)° after surgery. At the end of experiment, there were significant differences between experimental groups and control group in terms of Cobb angle (P = 0.007). CONCLUSION: The memory alloy staple implantation by anterior approach can prevent the growth direction of thoracic vertebrates from kyphosis.
