168例急性有机磷农药中毒对肾脏影响的临床探讨

Clinical Study of 168 Cases with Acute Organophosphate Pesticide Poisoning Effects on the Kidneys

目的探讨急性有机磷农药中毒对肾脏的影响。方法对168例急性有机磷农药中毒合并肾损害患者的年龄、中毒程度、肾损害及临床表现进行综合分析。结果急性有机磷农药中毒合并肾损害168例,占同期有机磷农药中毒患者的42.2%(168/398)。其中93例为重度中毒合并肾损害,占55.3%;61岁以上者占同期老年中毒患者的78.1%(25/32)。血尿、蛋白尿、氮质血症三者同时存在16例,死亡10例,病死率为56.2%。结论急性有机磷农药中毒肾损害易发生于重度中毒者及老年人,肾损害表现为单纯血尿、蛋白尿者预后良好,但如果合并氮质血症者病死率则显著增高。

Objective To investigate the acute organophosphorus pesticide poisoning（AOPP） on kidney. Methods 168 cases of acute organophosphate pesticide poisoning associated with renal damage in patients with age, degree of intoxication, renal damage and a comprehen- sive analysis of clinical manifestations. Results Acute organophosphorus pesticide poisoning associated with renal damage 168 cases, accounting for the same period in patients with organophosphate poisoning of 42.2%（ 168/398 ）. These 93 cases of severe poisoning associated with renal damage,accounting for 55.3% ;61 years or more for the same period and older poisoning patients from 78.1%（25/32）. Hematuria and proteinuria,azotemia three exist 16 cases,death 10 cases,the mortality was 56.2%. Conclusion Acute organophosphorus pesticide poisoning prone to kidney damage severe poisoning and the elderly, kidney damage manifested as simple hematuria, proteinuria good prognosis, but if the merger azotemia were significantly increased mortality.
