X线、CT及MRI对脊椎骨折的诊断价值

The Value of Spinal Fracture by X-ray,CT and MRI Methods

目的探讨X线平片、CT、MRI对脊椎骨折的不同诊断价值。方法收集40例脊椎骨折的X线、CT、MRI资料,分别对检出椎体及附件骨折、骨折片移位、椎管容积改变、脊髓损伤及韧带损伤的显示情况进行分析。结果X线平片可清晰显示骨折合并脱位者,发现较大的碎骨片,清晰地显示椎体的楔形改变。CT像可显示骨折部位及骨折的形态、方向,显示骨折碎片,尤其对椎管内碎骨片显示清晰,可发现附件骨折,显示椎管狭窄。MRI对脊髓损伤及其周围软组织的改变显示清晰。结论X线平片、CT、MRI对显示脊椎外伤骨折各有优缺点,多种影像方法结合能达到相互弥补不足的目的。

Objective To evaluate the different diagnostic values of X-ray,CT and MRI scan in spinal fracture. Methods The image （X-ray,CT and MRI）of 40 cases with vertebral fractures such as the fracture of vertebral body and accessory,the shift of fracture pieces,the change of spinal canal volume,the injury of spinal cord and ligament were analyzed. Results X-ray can show the fracture with dislocation clearly, find the larger fracture pieces and show the dovetail form of spinal body. CT can show the location,shape and orientation of fracture, especially fracture pieces in spinal canal, and find the fracture of vertebral accessory and show the narrowed spinal canal. MRI can show the injured spinal cord and soft tissue around it well. Conclusion X-ray,CT and MRI can show the vertebral fracture in different extent and each of the images can show different parts of the fracture. The Combination of X-ray, CT and MRI scan is important for diagnosis of spinal injury.
