肝细胞脂肪变与乙型肝炎相关性研究进展

Advances in researches on relationship of steatosis with hepatitis B virus infection

肝细胞脂肪变是慢性丙型肝炎(CHC)常见的病理表现,肝脂肪变的存在不仅影响CHC的持续病毒学应答(SVR),且与CHC疾病进展密切相关,但有关慢性乙肝(CHB)与肝细胞脂肪变相关研究不多。此文就肝细胞脂肪变在CHB流行现状、对肝细胞脂肪变发生机制及对CHB疾病进展的影响作一综述。

Hepatic steatosis is a common histological feature of hepatitis C and it less clear in chronic hepatitis B. Metabolic factors including obesity, WHR and dyslipidemia are associated with hepatic steatosis in chronic viral hepatitis. Insulin resistance may play an important role in the development of steatosis in chronic viral hepatitis C. In addition, HCV itself also has some effect on the development of steatosis in chronic hepatitis C, The natural history of chronic hepatitis B and C vary from asymptomatic chronic infection to fibrosis, cirrhosis and even hepatocellular carcinoma. It is reported that steatosis is associated with the progression of fibrosis in chronic hepatitis C. Some researches found that steatosis could negatively affect the response to the interferon-based antiviral treatment and may contribute to the liver carcinogenesis in chronic hepatitis C, This review discusses the viral and host factors associated with the steatosis in chronic viral hepatitis B. The relationship between steatosis and progression of fibrosis is also been discussed.
