充分血液透析对慢性肾功能衰竭患者肺弥散功能的影响

The Effects of Enough Hemodialysis on Lung Diffusion Function in Chronic Renal Failure Patients

目的：了解在充分透析情况下，慢性肾功能不全血透患者达到充分透析指标时和１ａ后肺弥散功能、通气功能和肺容量的变化。方法：测定１５例患者的Ｋｔ／Ｖ值、标准化蛋白质分解速率（ＮＰＣＲ）、尿素的时间平均浓度（ＴＡＣｕｒｅａ）及肺功能。结果：肺弥散功能大致正常的仅有４例，通气功能１１例减低，肺残气量１３例升高。本组１５例透析１ａ后ＦＶＣ有所增加，ＲＶ／ＴＬＣ下降，ＤＬＣＯ轻度下降，经统计学处理ＦＶＣ增高、ＲＶ／ＴＬＣ下降均无显著性差异（Ｐ＞０．０５），ＤＬＣＯ下降也无显著性差异（Ｐ＞０．０５）。结论：通过充分透析，肺通气功能和肺容量有所改善，肺弥散功能并无明显好转。

Objective:To study the change in lung diffusion function, ventilation function and lung volume of chronic renal failure patients who received enough hemodialysis.The data was obtained at the time of enough hemodialysis and after one year.Method:The Kt/V value,NPCR,TACurea and lung function of 15 patients were measured.Results:A normal lung diffusion function was found in 4 patients only,an impaired lung ventilation function in 11,and an increased lung residual volume in 13 patients.After one-year hemodialysis treatment,FVC in all 15 cases increased,RV/TLC decreased,DLCO decreased slightly,but the change in FVC,RV/TLC and DLCO had no statistic significant difference, P >0.05.Conclusion:The lung ventilation function and lung volume were improved after enough hemodialysis treatment,but no improvement was found in lung diffusion function.
