彩色多普勒在诊断甲状腺疾病中的应用

APPLICATION OF COLOR DOPPLER FLOW IMAGE IN DIAGNOSIS OF THYROID DISEASES

目的检测正常及各类异常甲状腺组织的血流动力学特征，探索彩超对甲状腺疾病的诊断价值。方法彩超对５０例正常及１２５例异常甲状腺（Ｇｒａｖｅｓ病３５例；结节性甲状腺肿３２例；腺瘤３８例；甲状腺炎１４例及腺癌６例）进行分析，测定其甲状腺上动脉（ＳＴＡ）流速、脉冲指数及阻力指数，并进行统计学分析。结果彩超诊断符合率９０．４％（１１３／１２５），Ｇｒａｖｅｓ病与腺癌和正常组比较流速升高（Ｐ＜０．０５），其它几类则无统计学差异。结论彩超是诊断甲状腺疾病及观察疗效的首选方法。

Objective The aim of this study is to investigate the hemodynamics features in normal and abnormal thyroid and to evaluate the diagnostic value of color Doppler Flow Image(CDFI). Methods CDFI was performed on 50 normal thyroids and 125 abnormal thyroids (Graves disease: 35; Nodular goiter: 32; adenoma: 38; thyroiditis: 14; adenocarcinoma: 6). The velocity, resistence index and pulsatility index of superior thyroid artery (STA) were measured. Results The reliability of CDFI is 90.4%. The blood flow and velocity of STA in Graves diseases and adenocarcinoma were greater than that of others (P<0.05). Conclusion CDFI of thyroid tissue is the first choice in the diagnosis of thyroid diseases.
