非侵袭型真菌性鼻窦炎的诊治经验

Experience of diagnosis and treatment for non-invasive nasal sinus mycoses

目的总结非侵袭型真菌性鼻窦炎的诊治经验，提高治疗效果。方法回顾性分析1995年1月至2005年12月收治的36例非侵袭型真菌性鼻窦炎患者的术前病情评估、治疗和术后随访资料。结果36例非侵袭型真菌性鼻窦炎患者，其中真菌球型25例均在鼻内镜下手术治疗，变态反应型11例，鼻内镜下彻底清除病灶，并I；3服给予强的松，鼻腔内局部皮质类固醇激素。随访2～6年，36例在随访期内未见复发。结论非侵袭型真菌性鼻窦炎分为两个亚型，即真菌球型、变态反应型。鼻内镜手术治疗为非侵袭型真菌性鼻窦炎首选治疗措施，其中变态反应型应综合治疗。

Objective To review the experience of diagnosis and treatment of the non-invasive nasal sinus mycoses. Method Medical history of 36 cases who were diagnosed as non-invasive nasal sinus mycoses were analyzed about the estimation before surgery, during the treatment and the follow-up after surgery between January 1995 and December 2005. Result Among 36 patients with non-invasive nasal sinus mycoses, the 25 cases of fungal ball were treated with endoscopic surgery. The 11 cases of allergic fungal sinusitis were treated with endoscopic surgery, after the surgery , they were treated with glucocorticoid. The period of follow -up was 2- 6 years. 36 cases had no recurrence during the follow-up period. Conclusion The non-invasive nasal sinus mycoses include fungal ball and allergic fungal sinusitis. Endoscopic surgery is the first choice for the non-invasive nasal sinus mycoses. And besides it allergic fungal sinusitis should be treated with other treatment.
