慢性心力衰竭患者QT离散度和心率变异性分析

Congestive Heart Failure Patients QT Dispersion and Heart Rate Variability Analysis

目的探讨慢性心力衰竭患者QT离散度和心率变异性变化特点及关系。方法56例心力衰竭患者和54例健康对照者为观察对象,全部入选者作静息状态下12导心电图和24h动态心电图,分析心衰组及健康对照组QT离散度及心率变异性。结果心衰组QT离散度为(48.4±17.45)ms,对照组为(27.87±10.74)ms,有显著性差异(P<0.001)。心衰组HRV各项指标较对照组明显减低(P<0.05￣0.01)。结论心衰时,由于自主神经受损,导致QT离散度及HRV异常;QT离散度和HRV改变与心衰程度明显相关。

Objective This study was aimed to investigate the characteristics and associations of QT dispersion and heart rate variability in congestive heart failure patients. Methods Fifty--six patients in congestive heart failure and fifty--four healthy controls were involed in the study --Resting electrocardiograms and 24--hour ambulatory electrocardiograms were recorded in all the patients and healthy controls. Analysed the QT dispersion and ItRV（included 24--hour time--domain,and non--linear analysis methods） of heart failure group and healthy controls group. Resutls The QT dispersion was （48.4 ± 17.45）ms in the heart failure patient, and （27.87±10.74）ms in the healthy controls（P 〈0.001）. Compared with the control group,the indications of HRV（SDNN. RMSSD.PNN50）were markedly and significantly reduced in head failure group（P 〈0.05-0.01）. Conclusion During the period of heart failure,the autonomic nervous were hurt to cause the change of the QT dispersion and HRV：The change of QT dispersion and HRV were significantly correlated with the degree of heart failure.
