绝经期骨性关节炎患者血浆性激素和细胞因子水平及其与膝关节功能的相关性 被引量:17

Correlation of Concentration of Gonadal Hormone and Cytokine with Knee Joint Function in Postmenopausal Women with Knee Osteoarthritis

摘要 目的分析绝经期妇女膝关节骨性关节炎(OA)患者血浆性激素和细胞因子水平及其与膝关节功能的相关性。方法选择近期在我院门诊就诊或住院治疗的绝经期膝关节OA患者89例,61例同期健康体检者为对照组,检测两组血浆性激素(雌二醇、睾酮和孕酮)和相关细胞因子(IL-1、IL-6、IL-10和TNF-α)水平,并用膝关节功能评定表(JOC)进行评估。结果绝经期膝关节OA患者的雌二醇、IL-1、IL-6、IL-10和TNF-α水平间差别均有统计学意义(P<0·05或<0·01)。绝经期膝关节OA患者的血浆雌二醇、IL-10水平与JOC多数得分及JOC总分呈正相关,而血浆IL-1、IL-6、TNF-α水平与JOC多数指标的得分及JOC总分呈负相关(P<0·05或<0·01)。结论绝经期膝关节OA患者雌激素水平明显降低,相关炎症因子明显增高,这些表现与膝关节功能相关。 Objective To analyze the correlation of the serum concentration of gonadal hormone and cytokine with the parameters of knee joint function in postmenopausal women with knee osteoarthritis (OA). Methods We enrolled 89 postmenopausal patients with knee OA (the OA group) and 61 healthy subjects (the control group), determined the serum gonadal hormone (aquadiol, andrusol and corporin) and the related cytokine (IL- 1, IL-6, IL- 10 and TNF- α), and assessed knee joint function by JOC. Results The level of dihydroxyestrin was significantly lower in the OA group than in the control group ( P 〈0. 01) ; the levels of IL - 1, IL -6 and TNF - α were higher in the OA group than in the control group, but the level of IL - 10 was lower in the OA group than in the control group ( P 〈 0. 01 or P 〈 0. 05 ) ; the levels of dihydroxyestrin and IL - 10 were positively correlated with the JOC scores of most dimensions and the gross scores of JOC, while the levels of IL - 1, IL - 6 and TNF - α were negatively related with the JOC scores of most dimensions and the gross scores of JOC ( P 〈 0. 01 or P 〈 0. 05 ). Conclusion In postmenopausal women with knee OA, there is a remarkable decrease in estrogen concentration and an obvious increase in related inflammatory factors, which is related with knee joint function.
