摘要 目的研究不同麻醉深度下异丙酚在犬脑不同区域的摄取和分布。方法12只雄性成年犬,随机分为浅麻醉组(S组)和深麻醉组(A组),分别以4.5mg/kg,7mg/kg的异丙酚静脉注射,达到预定麻醉深度后取颈内动脉和颈内静脉血并断头法将犬处死,解剖犬脑并分别取额叶、顶叶、颞叶、海马、扣带回、丘脑、中脑、桥脑、小脑等脑区组织。高效液相色谱-紫外线法(highperformance liquid chromatography,HPLC)测定血浆和脑组织的异丙酚浓度。结果S组异丙酚浓度丘脑(4.74±0.02)μg/g、桥脑(4.93±0.07)μg/g、中脑(4.80±0.02)μg/g,均明显高于其他脑区,而海马[(3.12±0.02)μg/g]最低(P<0.05);A组丘脑异丙酚浓度[(10.98±3.00)μg/g]明显高于其他脑区,海马[(5.59±0.75)μg/g]最低(P<0.05)。结论异丙酚在不同麻醉深度下不同脑区的摄取量不同,浅麻醉状态丘脑和脑干最高,深麻醉状态丘脑最高;两组海马最少。 Objective To investigate the cerebral uptake and regional distribution of propofol during different depth of propofol anesthesia at a single bolus in dogs. Methods Twelve male adult dogs were randomly divided into two groups and were injected right great saphenous vein with propofol 4.5 mg/kg (group S) and 7 mg/kg (group A) respectively in 15s. After effective anesthesia was established, blood samples were taken from the right carotid artery and right internal jugular vein for measurements of propofol concentrations in blood plasma by HPLC. Animals were sacrificed by decapitation. The frontal lobe, parietal lobe, temporal lobe, hippocampus, cingulate gyrus, thalamus, midbrain, pons, cerebellum were dissected for determination of propofol concentration by HPLC. Results In group S, propofol concentrations in the thalamus(4. 74 ±0. 02) μg/g, pons(4.93 ±0.07) μg/g and midbrain(4. 80 ±0. 02) μg/g were higher than those of other brain tissues, and was lowest in hippocampus ( 3. 12 ± 0.02 ) μg/g ( P 〈 0. 05 ) ; In group A, propofol concentration of cerebral tissues was highest in thalamus ( 10. 98 ± 3.00 ) μg/g, and lowest in hippocampus ( 5.59 ± 0. 75 ) μg/g ( P 〈 0. 05 ). Conclusion The propofol distribution during different depth of anesthesia at a single bolus in dogs is unequal. The propofol concentration is higher in thalamus and brain stem and is the lowest in hippocampus in both groups.
