腹部手术和开胸手术患者血乳酸和中心静脉血氧饱和度分析

摘要 目的观察腹部手术和开胸手术术后血乳酸和中心静脉血氧饱和度(ScvO2)变化。方法比较腹部手术组(33例)和开胸手术组(30例)术后1、6、24h血乳酸、ScvO2水平,以及术后并发症的发生情况。结果术后1h腹部手术组患者血乳酸水平明显低于开胸手术组,ScvO2明显高于开胸手术组,术后6、24h2组血乳酸、ScvO2水平无明显差异;腹部手术组术后1h内术后并发症发生率明显低于开胸手术组,术后6、24h并发症发生率无明显差别。结论开胸手术术后早期组织缺氧程度明显高于腹部手术,并导致术后早期并发症发生率增高,随着时间推移组织缺氧程度逐步减轻。 Objective To observe the levels of lactate and ScvO2 between patients after abdominal operation and thoracotomy. Methods Levels of lactate and ScvO2 in 33 patients after abdominal operation and 30 patients after thoracotomy were observed at the 1st, the 6th and the 24th hour after operation. Results The lactate level of patients after abdominal operation was lower than that of patients after thoracotomy, and level of ScvO2 was greater at the 1st hour after operation. But there was no obvious difference between the two groups at the 6th and the 24th hour after operation. As for the incidence of postoperative complications, patients after abdominal operation had fewer complications than the other group at the 1st hour after operation. But no obvious difference was found between the two groups at the 6th and the 24th hour after operation. Conclusion The early hypoxia in patients after thoracotomy is greater than that in patients after abdominal operation, but the difference will be eliminated with suitable treatment.
