腹腔镜筋膜内子宫切除术的临床应用体会

The clinical application of laparoscopic intrafascial supracervical hysterectomy

目的:总结腹腔镜筋膜内子宫切除术(laparoscopic intrafascial supracervical hysterectomy,LISH)临床应用及预防并发症的体会。方法:回顾分析1999年10月至2007年5月我院妇产科为258例患者施行LISH的临床资料。结果:手术开展初期发生合并症18例,15例及时采取措施治愈,3例中转开腹治愈,余手术效果满意。结论:LISH安全,副损伤小,术后性功能、膀胱直肠功能及盆底组织不受影响,可防止宫颈癌的发生。熟练掌握腹腔镜技术,慎重选择病例、及时中转开腹是防治并发症发生的关键。

Objective：To summarize the experience of clinical application and prevention of complications of laparoscopic intrafascial supracervical hysterectomy（LISH）. Methods：The clinical data of 258 cases underwent LISH in our units from December 1999 to May 2001 was analysed retrospectively. Results：The effect of LISH was satisfactory for that 18 cases expressed complications at the early stage of which 15 cases were cured timely and 3 cases were converted to laparotomy. Conclusions：LISH will be used more wildly for the benefits of safety,less injury,no harm to sexual function,urinary bladderrectum function and pelvic diaphragm formation. It also can be used to prevent or cure the cervical cancer. Pratised laparoscopic procedure,careful case selection and timely laparotomy might avoid the occurence of complications.
