瑞芬太尼诱导期急性高容性血液稀释在老年患者的应用

摘要 目的观察瑞芬太尼诱导期急性高容性血液稀释(AHH)对老年患者血流动力学及动脉血气,电解质的影响。方法印例拟行异丙酚,瑞芬太尼麻醉诱导的老年患者随机分为6%羟乙基淀粉(HES)组(Ⅰ组,n=30)及醋酸钠林格氏液(勃脉力A)组(Ⅱ组,n=30),Ⅰ组在麻醉诱导前输入HES8ml/kg,诱导后继续输入HES达12ml/kg行AHH;Ⅱ组以同样方法输入勃脉力A。分别记录基础值(Tn)、诱导后插管前(T1)、插管后即刻(T2)、10min(T3)、20min(T4)、切皮前的平均动脉压〈MAP)、心率(HR)、脉搏氧饱和度(SPO2)和PETCO2;于AHH前(t0)、AHH后即刻(t1),AHH后1h(t2)经桡动脉抽血测Hh,Hct及动脉血气,电解质。结果Ⅰ组、Ⅱ组的血流动力学稳定,SPO2,动脉血气,电解质差异无统计学意义。扩容时间Ⅰ组〉Ⅱ组。结论晶体液和胶体液扩容预防瑞芬太尼全麻诱导期低血压效果无明显差别,用于诱导期扩容安全有效,而HES扩容作用及维持时间优于勃脉力A。
