摘要
目的探讨一氧化氮(NO)对大鼠无心跳供体(NHBD)肺缺血再灌注损伤(IRI)的影响。方法大鼠随机分为Ⅰ组:有心跳供体(HBD)组;Ⅱ组:NHBD-吸氧组;Ⅲ组:NHBD-吸氧+NO组。Ⅰ组在处死的同时灌注4℃低钾右旋糖苷液(LPD),Ⅱ组、Ⅲ组处死后维持辅助呼吸,放置室温中30 min再灌注LPD液。供肺置于4℃LPD液中4 h后行原位左肺移植术。术后维持辅助呼吸1h。Ⅲ组通气吸氧的同时吸入浓度为40×10-6的NO。结果与Ⅱ组比较,Ⅲ组肺顺应性、动脉血氧分压高,肺组织丙二醛含量低(P<0.05)。结论吸入低浓度NO可减轻大鼠NHBD肺缺血再灌注损伤。
Objective To evaluate the effect of inhaled nitric oxide(NO) on the ischemia-reperfu-sion injury(IRI) in rat lungs from non-heart-beating donors(NHED). Methods Sixty Sprague-Dawley rats were randomly divided into 3 groups: group Ⅰ, heart-beating donor; group Ⅱ , NHBD with 30 minutes of warm ischemia time(WIT) and oxygen inhaled;group Ⅲ ,NHBD with 30 minutes of WIT and oxygen and NO inhaled. The donor lungs of group I were flushed with low potassium dextran(LPD) solutlon at 4 ℃ after asystolia while the lungs of group Ⅱ and group Ⅲ remained ventilated at the room tern-perature for 30 minutes after asystolia and then were flushed with LPD solution. All of the donor lungs remained inflated when they were stored in LPD solution at 4℃ for 4 hours. The recipient rats under-went orlhotopic left lung lransplamation. After transplantation I hour's reperfusion and ventilation was manipulated. The donor hmgs of group Ⅲ inhaled additional NO with the concentration of 40×10^-6 at the time of ventilation. Results Compared with the group Ⅱ , the pulmonary compliance and the oxygen partial pressure of the arterial blood are higher, the MDA in the lung lower(P〈0.05). Conclusion The low-dosage of NO is an effective treatment to attenuate the acute injury resulted from the ischemia-reperfusion in the rat lungs from non heart beating donors.
