摘要 目的比较不同屈光度、年龄、性别双眼角膜地形图对称性差异的特征。设计回顾性病例系列。研究对象拟行屈光性角膜手术的年龄18~47岁的近视眼患者500例(1000眼)。方法对受试者双眼行角膜地形图(TOMEY Version 3.30)检查。检查前停戴角膜接触镜1周以上,并排除干眼和其它眼病及全身系统疾病。主要指标角膜地形图对称性、对称比率。结果最大散光轴位和屈光度同侧一致3.4%,基本对称1.8%,近乎对称79.0%,不对称15.8%。不同屈光度对称比率<-6.0D占12.0%,-6.0~-7.9D占24.0%,-8.0~-9.9D占25.8%,≥10.0D占19.0%(P<0.05)。不同年龄对称比例<35岁占84.3%,≥35岁68.5%(P<0.05)。男性对称比率占86.4%,女性76.7%(P>0.05)。结论80%的近视患者双眼角膜地形图呈对称性分布。屈光度越高(-10.0D以内)对称性越高;随年龄增长对称性逐渐降低。(眼科,2008,17:40-43) Objective To compare the bilateral symmetry characters of corneal topography in patients with different refraction, age and gender. Design Retrospective case series. Participants 1000 eyes of 500 patients who were going to undergo corneal refractive surgery with myopia from 18 to 47 years old. Methods Bilateral corneal topography was obtained using TOMEY Version 3.30. Contact lenses had not been worn for more than a week before the examination, and patients with dry eye or other ocular and systemic diseases were excluded. Main Outcome Measures Symmetry and symmetric rates of corneal topography. Results Bilateral symmetry of location of the cone apex and refraction: uniform 3.4%; almost symmetry 1.8%; approximately symmetry 79.0%; asymmetry 15.8%. The rates of bilateral symmetry in patients with different refraction: 〈-6.0D,12.0%; -6.0- -7.9D, 24.0%; -8.0-9.9, 25.8%; ≥ 10.0D, 19.0% (P〈 0.05). The rates of bilateral symmetry in patients with different age: 〈35 years, 84.3%; ≥ 35 years, 68.5% (P〈0.05). The rates of bilateral symmetry in patients with different gender: male, 86.4%; female, 76.7% (P〉0.05). Conclusions 80% of patients with myopia have bilateral symmetric corneal topography patterns. The rates of bilateral symmetry of corneal topography increase with the increase of spherical refraction (〈-10.0D) and decline with the increase of age.
