摘要
目的:探讨CT、超声和骨显像的联合应用在诊断神经母细胞瘤骨骼及其他组织脏器转移及临床分期中的价值。材料和方法:43例神经母细胞瘤患儿,行CT、全身骨显像和超声检查。结果:43例患儿中,CT示大小不等、密度不均的肿块,10例侵犯椎管腔,20例包绕大血管、肿块生长过中线,后腹膜淋巴结肿大17例,肝脏受累7例,肾脏受累9例,头颅转移10例;超声示大小不等、回声不均匀的实质肿块34例,其中20例包绕腹部大血管,过中线,后腹膜淋巴结肿大20例,肝脏受累9例,脾脏受累2例,肾脏受累9例;骨显像中原发灶显影23例,颅骨、脊柱、四肢、肋骨、骨盆受累分别为14例、16例、12例、23例和10例。结论;影像学联合检查对诊断NB、有无转移及临床分期有相当大的价值。
Purpose： To evaluate the joint application of CT , ultrasound and bone scan in the diagnosis of NB , metastasis and its clinical staging.Materials and Methods：Ultrasound, Bone scan and CT were performed in 31 NB patients. Results： 43 CT positive cases showed soft tissue mass with different size and heterogeneous attenuation. There were intraspinal extension in 10 cases, main vessel embraced in 20 cases, lymphadenopathy in 17, liver involvement in 7 cases, renal involvement in 9 cases, skull metastasis in 10 cases. Among the 43 cases involved, 34 cases show hetero - echogeneous mass, in which 20 main abdomen vessel embraced and exceed the middle line, 20 cases show attenuation of the posterior peritoneal lymph node, 9 cases with liver involvement, 2 cases with spleen involvement and 9 cases with renal involvement. 23 of 43 cases were primary tumor observed in bone scan. Skull, spine, extremities, rib and pelvis involvements were 14, 16, 12, 23 and 10 respectively. Conclusion： Bone scanning and CT are of great value in the diagnosis of NB, its metastases and clinical staging.
