摘要
目的探讨彩色多普勒超声(CDFI)监测脐动脉(UmA)及大脑中动脉(MCA)血流预测胎儿宫内缺氧的价值。方法运用CDFI产前监测孕晚期胎儿MCA及UmA血流,测量收缩期血流速度(Vs),舒张末期血流速度(Vd)、Vs和Vd比值(S/D)、搏动指数(PI)和阻力指数(RI),并与妊娠结果对照。结果正常妊娠时随孕周增加UmA、MCA的S/D及RI逐渐下降,异常妊娠时UmA的S/D及RI均明显高于正常组,而MCA的S/D及RI明显低于正常组,当胎儿MCA血流S/D<4、PI<1.6、RI<0.6时,提示胎儿宫内缺氧。结论CDFI联合检测胎儿MCA及UmA的S/D、PI及RI对预测胎儿宫内缺氧有重要价值。
Objective To evaluate the clinical value of CDFI in diagnosing intrauterine hypoxia by monitoring blood flow in umbilical arteries and cerebral arteries. Methods Color Dopper blood flow was used to monitor antepartum MCA and UmA blood flow and measure VS, VD, S/D value, PI and RI. Results S/D value of UmA and MCA decreased when pregnant time increased in normal pregnancy. S/D value of UmA and its RI value in abnormal pregnancy was obviously higer than those of the control normal group, while S/D value of MCA and its RI value in abnormal pregnancy obviously lower. When S/D 〈 4. 0, PI 〈 1.6 and RI 〈 0. 6, intrauterine hypoxia was indicated, showing that under the circumstance of abnormal pregnancy, RI would decrease first to ensure the blood supply to cerebra. Conclusion Combined measuring of S/D value, PI and RI can help diagnose intrauterine hypoxia.
