血小板活化因子与P-选择素在脑梗死中的作用

摘要 目的探讨血小板活化因子(PAF)与P-选择素(CD62p)在脑梗死中的作用。方法用ELISA法测定50例急性期脑梗死(其中20例2周后复查)患者及20例健康体检者外周血PAF、CD62p水平,同时测定外周血白细胞计数、中性粒细胞百分比及血小板参数,并记录头部CT所示梗死体积及部位,测定神经功能评分,观察其在脑梗死不同时期的变化并探讨其临床意义。结果①脑梗死患者急性期外周血PAF、CD62p水平明显升高,恢复期有所下降,但仍高于健康对照组(P<0.01);脑血栓形成较腔隙性脑梗死PAF、CD62p水平明显升高(P<0.01);②PAF与CD62p水平与外周血白细胞计数(WBC)、中性粒细胞百分比(N%)、平均血小板体积(MPV)、梗死体积、收缩压及神经功能评分呈正相关;与血小板计数(BPC)呈负相关,且PAF与CD62p之间呈正相关;③不同梗死部位二者无显著性差异(P>0.05)。结论血小板活化因子及P-选择素与急性脑梗死关系密切。 Objective To investigate the effects of platelet activating factor(PAF) and P-selectin(CD62p) in cerebral infarction.Methods The levels of PAF,CD62p in peripheral blood of 50 patients with cerebral infarction in acute stage(20 patients had been rechecked 2 weeks later) and 20 healthy persons and the leucocyte count,the neutrophil percentage and the platelet parameter were examined by ELISA,the infarct size and position were tested.The nervous function was evaluated,and the changes of the 2 factors in different timepoints of cerebral infarction were observed and the clinical significance was analyzed.Results The levels of PAF and CD62p in peripheral blood of cerebral infarction in acute stage increased obviously and decreased in restoration stage,but still higher than that in the control group(P〈0.01);the levels of PAF and CD62p in peripheral blood of cerebral infarction were higher than that of the lacunar infarction(P〈0.01).The levels of PAF and CD62p had positive correlations with leucocyte count,the neutrophil of percentage,mean platelet volume(MPV),contractive pressure,infarct size and nervous function score;had inverse correlation with blood platelets count;further had positive correlations with both PAF and CD62p,but had no relation with infarction locus(P〉0.05).Conclusion PAF and CD62p closely correlate with acute cerebral infarction.
