摘要
[目的]探讨社会心理因素与乳腺肿瘤患者免疫功能状态的相关性。[方法]应用流式细胞仪检测61例乳腺肿瘤患者(恶性肿瘤患者40例,良性肿瘤患者21例)、43例健康对照者外周血中T淋巴细胞亚群水平;应用临床心理问卷(EPQ、SSRS、ENRICH)进行评定。采用F检验、person相关分析。[结果]3组被试免疫指标(CD3+细胞、CD4T细胞、CD8T细胞、CD4+/CD8+比值、NK细胞)、EPQ的E、N两个分量表值、SSRS的各项(主观支持、客观支持、对支持的利用度、总分)测定值、ENRICH在婚姻满意度、性格相容度、夫妻交流、解决冲突的方式、业余活动、性生活和角色平等性7个分量表的测定值比较差异有统计学意义(P﹤0.05或﹤0.01)。EPQ的P量表分与CD4T细胞值、E量表分与NK细胞值、N量表分与CD4+/CD8+比值、N量表分与NK细胞值呈显著相关,SSRS的主观支持、对支持的利用度、SSRS的总分均与NK细胞值呈显著相关(P﹤0.05或﹤0.01)。[结论]乳腺癌患者存在较严重的心理问题;免疫功能低下,免疫功能受各种心理社会因素的影响。
[Objective] To explore the correlation of immune function of patients with breast cancer and social psychic factors. [Methods] The total T lymphocytes (CD3+cells) and T lymphocytes subsets (CD3+CD4+cel/s and CD3+CD8+cells) were detected by flow cytometry in the peripheral blood of patients with breast cancer (n=40), patients with benign tumor (n=21) and healthy persons (n = 43) as control, and the ratio of CD3+CD4+/CD3+CD8+ was calculated. At the same time. the patients with breast cancer, benign tumor and healthy persons were measured by clinical psychological rating scale (EPQ, SSRS, ENRICH). [Results] T lymphocyte subsets (CD3+, CD4, CD8, CD4+/CD8+, NK), EPQ (EPQ-E, EPQ-N), SSRS and ENRICH (the score of satisfaction of marital life. compatible degree of character, communieation, fashion of solving problem, leisure activities, spouses and quality of sexual life) in three groups were remarkably different (P 〈 0.05, 〈 0.01 ). There was a significant relationship between EPQ-P and CD4. EPQ-E and NK. EPQ-N and CD4+/CD8+, EPQ-N and NK, the subjective support and NK, the utility of social support and NK, the total scores of social support and NK (P 〈 0.05 or 〈 0.01). [Conclusion] The patients with breast cancer have more server psychosocial problems and lower immune function. The immune function of patients with breast cancer before operation is associated with the various psychosocial factors.
