气管内镜在鼻内镜系统下结合电动切割器行腺样体切除50例

The Adenoidectomy under the Bronchoscope Against the Nasal Endoscope and Combined with the Powered Microdebride

目的探讨气管内镜在鼻内镜系统下结合电动切割器行腺样体切除的实用价值。方法50例患者在全身麻醉下行鼻内镜系统下直接接0°、直径2．7min、长30cm的气管内镜在电视监视器下，结合美国Stryker电动切割器行腺样体切除术。结果手术过程中无一例患者行下鼻甲部分切除，术中患者出血少，约15～30mL，术后创面用盐酸羟甲唑啉喷雾剂收缩无出血后，粘贴止血绫，术后无出血现象。腺样体切除手术时间5～15min，无术后并发症出现。结论气管内镜在鼻内镜系统监视下，结合电动切割器行腺样体切除，简单易行，经济实惠。

Objective To describe the cost -effective of adenoidectomy under the bronchoscope against the nasal endoscope and combined with the powered microdebrider. Method fifty patients were undergone adenoidectomy under the TV monitor. Intra -operatively and against the nasal endoscope, hard bronchoscope, with 0° in temperature, 2. 7mm in diameter and 30cm in length, were directly combined with the powered microdebrider produced by Stryker of the USA. Result No patient have to partly resect the inferior turbinate during the operation and meanwhile the blood loss is as little as about 15 - 30mL. After the operation, the wound part was shrink with Oxymetazoline Hydrochloride Spray till the blood loss was over. Then stick stanching satin. The surgical time was 5 - 15min. No patient complained of blood bleed and other complications postoperatively. Conclusion Under the watch of nasal endoscope, the adenoidectomy by the bronchoscope, combined with the powered microdebrider, has solve the problem of difficult operation and unclean resection of the adenoidectomy under the general na- sal endoscope, which was caused by the angle because of the collision caused by the powered microdebrider and hard endoscope. Such adenoidectomy is a easy-performed, cost- effec- tive surgical procedure.
