房室延迟、室间延迟和室内延迟对心功能影响的研究现状

摘要 房室延迟(AVD)、室间延迟(VVD)和室内延迟(IVD)是起搏器置入患者术后程控的三个重要间期。目前最佳AVD、VVD和IVD的获得已经历了从观察体表心电图上QRS波时限的变化到通过观察超声机械运动指标的演变,甚至有学者提出用无创性血压监测来达到优化AV间期的目的。通过调节AVD、VVD、IVD可获取最好的心功能。而对于一个电机械延迟藕联或闰盘损伤的患者,调节AVD、VVD和IVD的效应有限。最佳AVD、VVD、IVD对急性期心功能临床获益的证据较多,而对中远期心功能的影响尚处于探索阶段。
