腹腔镜胆囊切除术中Calot三角内变异肝右动脉的辨识与保护

Identification and protection of abnormal right liver artery during Calot′ traingle dissection in LC

目的探讨腹腔镜胆囊切除术(Laparoscopic cholecystectomy,LC)中Calot三角内变异肝右动脉的辨识与保护方法,避免误损肝右动脉,降低LC并发症的发生率。方法回顾性分析笔者于2004年12月～2007年6月行LC时,所发现20例在Calot三角内存在有变异肝右动脉的病例的临床资料。结果20例Calot三角内变异肝右动脉均得以准确辨识和保护,均成功施行LC,无1例发生肝右动脉损伤。结论熟识肝右动脉的变异类型并采用恰当的解剖Calot三角方法,是避免LC术中误损变异肝右动脉的关键。

[Objective] To discuss the identification and protection for abnormal right liver artery during Calot＇ traingle dissection in LC in order to avoid injurying right liver artery and decreasing sugery complication. [Meth- ods] Retrospectively reviewed the clinical data of 20 patients with abnormal right liver artery in calot＇ traingle found during LC from December 2004 to June 2007. [Result] All the 20 cases bad LC successfully and all the abnormal right liver arteries were well identified and protected during LC. [Conclusion] The key to avoid injurying right liver artery during LC is well acknowledged the abnormal types of right liver artery and dissecting calot＇ traingle in appropriate way.
