同期双膝关节表面置换治疗膝骨关节病

Simultaneous Total Knee Arthrosis Surface Replacement to Treat Arthropathy of Knee

目的探讨同期双膝关节表面置换术治疗膝骨关节病的安全性和临床疗效。方法将11例严重双膝骨关节病在同次手术施行双侧膝关节表面置换的患者作为Ⅰ组,以同期施行的18例单膝关节置换术作为Ⅱ组。结果术后经6个月～4年,平均18个月的随诊,两组患者在术后膝关节功能改善,疼痛减轻和伤口愈合方面没有明显的差别。Ⅰ组患者平均单侧医疗费用要明显低于Ⅱ组。Ⅰ组出现深静脉血栓2例,Ⅱ组发生深静脉血栓1例。结论下肢同期双膝关节置换,对于严重的下肢多发畸形的治疗,具有省时、康复方便、见效快等优点。但难度大,并发症发生率高,应严格掌握适应证,认真做好术前准备。

Objective To discuss security and clinic effect of simultaneous total knee arthrosis surface replacement treatment on otsteoarthropathia of knee. Methods Put 11 cases of otsteoarthropathia of total knee into Ⅰ group, and put 18 cases single knee arthrosis surface replacement surgery treatment on otsteoarthropathia of knee into Ⅱ group. Results From 6 months to 4 year＇ s observation, average 18 months treatment, both groups are well, there is no difference about pain alleviatation and wound healing. But patient of Ⅰ group＇ s expending is lower than that of Ⅱ group. Two cases vein thrombus happened in Ⅰ group,in the Ⅱ group happened 1 case. Conclusion Simultaneous total knee arthrosis surface replacement surgery treatment on otsteoarthropathia of knee have more merit, including time ecnomization,healing convenience and quik effect. But the difficulty is high, complication happened frequently, we must be prepared well before surgery.
