摘要
目的:应用新版血管回声跟踪(E-Tracking)技术评价肝硬化患者颈总动脉血管弹性。方法:分别对47例肝炎后肝硬化患者(平均年龄50.8岁)和68例正常健康者(平均年龄48.3岁)采用线阵高频探头检测静息状态下颈总动脉(CCA)内中膜厚度(IMT),应用新版血管回声跟踪技术检测弹性系数(Eρ)、硬化参数(β)、顺应性(AC)、单点脉搏波传导指数(PWVβ)和增大指数(AI),并对肝硬化患者组上述参数进行相关性分析。结果:肝硬化组较正常对照组Eρ、β、PWVβ及IMT增高,AC降低,且均具有显著性差异(P<0.05或P<0.01)。肝硬化组的PWVβ与Eρ、β及IMT呈正相关,与AC呈负相关;肝硬化组AI与Eρ、β、PWVβ、AC及IMT无明显相关性。结论:新版E-Tracking技术客观反映了肝硬化患者外周血管的改变,是评价血管内皮功能简便、实用的方法。
Objective : To study the wall elasticity of the carotid artery using newly Echo-tracking in liver cirrhosis. Methods: 105 cases were divided into 2 groups:47 cases in the group of liver cirrhosis and 68 cases in the group of normal adults, Newly Echo-tracking was used to evaluate the carotid elastic modulus, include stiffness parameter (13), the pressure-strain elastic modulus (Ep), arterial compliance (AC), one-point pulse wave velocity(PWVβ),augmentation index(AI) and analyse the correlation. Results: Compared with the control group,data of Ep,β,PWVβ,IMT were significantly higher while AC was obviously lower (P〈0. 05 or P 〈0. 01);AI in two groups have no significant difference; In liver cirrhosis group, PWVβ was positively correlated with β,Ep, IMT and negatively correlated with AC;While there was no correlation among β, Ep, IMT,AC and AI ;Conclusion : Newly E-Tracking technique can be used to reflect the vascular changes of liver cirrhosis and it is a convenient,practical method to evaluate vascular endothelial function.
