肋间神经外侧皮支营养皮瓣的相关研究

The study of neurocutaneous vascular flap ( NCVF) with lateral cutaneous branches

目的探讨季肋部皮神经营养血管皮瓣可行性。方法在12例24侧成人躯干标本上，观察了第7～11肋间神经外侧皮支及其营养血管的来源、走行及分布规律；在临床上，应用第9肋间神经外侧皮支皮神经营养血管皮瓣修复组织缺损5例。结果第7～11肋间神经外侧皮支从腋中线前后相应肋间穿出深筋膜，分为前、后支，呈节段性、重叠性分布于侧胸腹部皮肤。皮神经营养血管相互间形成丰富的吻合。临床应用取得了满意的疗效。结论以第9、10肋间神经外侧皮支营养血管为蒂．可以设计皮神经营养血管皮瓣。

Objective To provide anatomical basis for NCVF with lateral cutaneous branches （LCBs） of T7 - Tn and their nutrient vessels originating from its pedicle. Methods The origin, direction and distribution of LCBs of T7 - T11 and their nutrient vessels were observed in 12 adult specimens. Results LCBs of T7 - T11 penetrated through their respective intercostal tissues near midaxilla line. Each LCB was divided into two opposite branches. There were copious anastomoses among these vessels and between nutrient vessels of two neighboring cutaneous nerves. The NCVF based on lateral cutaneous branch of a single intercostal neurovascular bundle was applied for tissue repair in 5 patients. The operations were successful. Conclusion Flap can be designed with LCBs of T9 , T10 carrying its nutrient vessels from its pedicle.
