阿奇霉素对支气管哮喘患儿外周血辅助性T淋巴细胞功能的影响 被引量:15

摘要 目的探讨阿奇霉素对支气管哮喘患儿外周血辅助性T淋巴细胞(Th)1/Th2功能的影响。方法选取24例支气管哮喘发作期患儿为支气管哮喘组,20例健康儿童为健康对照组。二组均在无菌条件下获取外周血单个核细胞(PBMC)悬液,哮喘组予不同质量浓度(0.2、0.1、0.05、0mg/L)阿奇霉素治疗,健康对照组未予阿奇霉素干预,均体外培养48h。酶联免疫吸附(ELISA)法检测培养上清液中干扰素-γ(IFN-γ)、IL-4及IL-10水平变化。采用SPSS11.5软件进行统计学分析。结果1.支气管哮喘组患儿PBMC产生IL-4水平明显高于健康对照组(P<0.05),IFN-γ/IL-4比值明显低于健康对照组(P<0.05),IFN-γ和IL-10水平与健康对照组比较无显著性差异(Pa>0.05);2.支气管哮喘组患儿PBMC在阿奇霉素0.1mg/L水平条件下,IL-4分泌水平明显高于0.2mg/L和0.05mg/L组(Pa<0.05)及空白对照组(P<0.01),各组间PBMC分泌IFN-γ水平和IFN-γ/IL-4比值均无显著性差异(Pa>0.05);3.阿奇霉素0.2mg/L和0.1mg/L组PBMC产生IL-10水平均明显高于空白对照组(P<0.01,0.05),0.2mg/L组明显高于0.05mg/L组(P<0.05),其他各组间均无显著性差异(Pa>0.05)。结论常规质量浓度阿奇霉素(0.1mg/L)对支气管哮喘患儿外周血Th1/Th2平衡无明显影响,但可通过上调PBMC产生IL-10水平,而间接发挥其免疫调节作用。 Objective To explore the influence of Azithromycin on helper T lymphocyte cell(Th) 1/Th2 in peripheral blood of children with bronchitic asthma. Methods Twenty - four asthmatic children and 20 healthy children were selected. Peripheral blood mononuclear cells (PBMC) were isolated from venous blood and made into cells suspension in aseptic condition. 0.2 mg/L,0.1 mg/L,0.05 mg/L and 0 mg/L Azithromycin were added into the cultures in asthmatic group. The control group was not interfered with Azithromycin. The supernatant was col- lected after 48 h. The levels of IFN -γ, IL -4 and IL - 10 in the supernatant were determined by enzyme - linked immunosobent assay (ELISA). SPSS 11.5 software was used to analyze data. Results 1. The level of IL - 4 produced from PBMC of asthmatic group was significantly higher than that of control group ( P 〈 0. 05 ), but IFN-γ/IL - 4 of the former was obviously lower than that of control group ( P 〈 0. 05 ). There were no significant differences of the level of IFN - 31 and IL - 10 between the 2 groups (Pa 〉 0.05 ). 2. Azithromycin 0.1 mg/L more promoted the secretion of IL - 4 than the other 3 concentrations ( Pa 〈 0.05,0.01 ), and there were no significant differences of the level of IFN - 3, or IFN -γ/IL -4 between any other 2 groups (Pa 〉0.05). 3. Azithromycin 0.2 mg/L and 0.1 mg/L more increased the level of IL - 10 than the control group(P 〈0.01,0.05) ,and the level of IL - 10 of 0.2 mg/L asthmatic group was higher than that of 0.05 mg/L group( P 〈 0. 05 ). There were no significant differences between any other 2 groups ( Pa 〉 0.05 ). Conclusions The routine drug level of Azlthromycin(0.1 mg/L) had no effects on the imbalance of Th1/Th2 of asthmatic children, but could modulate the immunological function by up - regulating the level of IL - 10.
