尿转铁蛋白、微量白蛋白和β_2微球蛋白在过敏性紫癜患儿早期肾损伤中的变化 被引量:6

摘要 目的探讨尿TRF、MA及β2-MG与过敏性紫癜肾炎发病的关系,探寻过敏性紫癜(HSP)早期肾损伤敏感的实验室指标。方法过敏性紫癜患儿分为皮肤型组、混合型组、HSPN型组、健康对照组和治疗后组。采用放射免疫分析法测定尿TRF、MA及β2-MG。结果HSP早期各临床分型比较发现HSPN型组尿TRF、MA及β2-MG水平分别高于其他临床型组(P<0.05);治疗后组3个指标都高于对照组(P<0.05),有显著性差异。HSP早期患儿尿TRF、MA及β2-MG水平明显增高,伴有肾损伤者升高尤为明显,恢复期有所降低但仍处于较高水平。结论尿TRF、MA及β2-MG可作为反映早期HSP肾损伤的指标。 Objective To detect and explore the changes of urine transferrin (TRF), urine microalbumin (MA) and β2-microglobulin(β2-MG)in children with Henoch-Schenlein Purpura (HSP). Methods Children involved in the experiments were divided into three groups: HSP group; ( HSP were divided into pure purpura group), mixed pattern group and HSPN group; normal control group; recovery stage group. This experiment was detected TRF, MA and β2-MG by radioimmunoassay. Results HSP comparison of clinical classification showed that the urine TRF , MA and β2-MG level in purpura nephropathy were higher than that in any others(P 〈0.05) , the three indexes were higher in after-treated group than controls(P 〈 0.05); abnormality ratio of TRF, MA and β2-MG were respectively higher than BUN/Cr, urine analysis, BUN/Cr and urine analysis(P 〈0.05) ; there was obvious difference. Conclusion The average levels of urine TRF, MA and β2-MG are increased significantly particularly with early renal damage in HSP patients. Urine TRF, MA and β2-MG are sensitive markers of renal damage in early stage of HSP patients.
