人工耳蜗植入前电诱发听性脑干反应测试的临床应用 被引量:6

摘要 目的探讨人工耳蜗植入前电诱发听性脑干反应测试的记录方法及意义。方法2例无残存听力的耳聋患者,在人工耳蜗植入术中,植入装置前,应用改装的人工耳蜗植入体连接体外言语处理器作为电刺激仪,2根自制的铂铱合金球形电极作为刺激电极,置入造孔后的鼓阶内,以电荷平衡双相脉冲电流进行电刺激,并用GSIAudera诱发电位仪记录电诱发听性脑干反应结果。结果2例患者均记录到分化良好的电刺激听性脑干反应波形,并顺利植入人工耳蜗,术后获得良好的听力受益。结论人工耳蜗植入前进行电诱发听性脑干反应测试能够较准确地反映听觉传导通路功能完整性,有利于判断人工耳蜗植入后患者能否获得听性反应。 Objective To evaluate the method of recording electrically evoked auditory brainstem responses (EABR) before cochlear implantation. Methods Two deaf patients received cochlear implantation. Prior to the operation, a modified cochlear implant attached to an in vitro speech processor was used as the electrical stimulation device while two homemade spherical platinum-- iridium electrodes were placed into stomatal scala tympani as the stimulation electrodes. Charge--balanced biphasic constant current was set as the stimulation pulse, and GSI Audera evoked potential instrument recorded evoked auditory brainstem response. Results The EABR waveforms were well differentiated in both patients. After a successful cochlear implantation, the patients reported improved audito- ry abilities. Conclusion The application of EABR is considered as an objective electrophysiological method that can accurately monitor the functionality of the auditory pathway. It has important clinical value in predicting whether patients can acquire auditory responses with the aid of cochlear implantation.
